Our Allied Health referral partners educate participant and family members on eating well. We make sure that diets meet individual needs. Our Support staff follow the menus to meet the participants nutritional needs.
Hospital Discharge Support
Hospital Discharge Support is rapid response service which provides support to you in the community. It will help you recover from a recent illness or surgery.
Our Allied Health referral partners will help you in your home and follow the care plan which is designed to achieve your goals. Our aim is to get you back on your feet and help you avoid a return to hospital.
High Intensity
We understand that when people have complex health conditions, they may require special care and support. Our trained and experienced support staff can provide a wide range of supports during the day and night.
if the staff need any extra training for any specific needs, we provide all additional training they may need. Our participants get to meet the Nurses and Support staff before they start, just to ensure that they not only have the right skill set but is the right person to support you.
Care Plans are prepared by our Registered Nurses (RN) to meet the needs and preferences of the individual. We encourage our participant to take complete control of their service, making choice and remaining active where possible.
